Op Report 12

DIAGNOSES: 1. Cervicalgia. 2. Cervicogenic headaches. 3. Fibromyalgia.

ANESTHESIA: Intravenous sedation

NAME OF OPERATION: 1. Median branch blocks at C3, C4, C5, C6, and C7 levels, right side. 2. Monitored intravenous Versed sedation.

PROCEDURE: The patient was taken to the block room. She was placed supine. She was monitored appropriately. She was administered Versed 2 mg IV. Her neck was prepped and draped. The C-arm was brought in. The articular pillars at C-3, 4, 5, 6, and 7 on the right were visualized. After adequate local anesthesia, 25-gauge spinal needles were inserted at each of these levels, and came in contact with midpoint of the articular pillar where the median branch was located. No paresthesias were noted. There was negative aspiration. Each level was injected with 20 mg Depo-Medrol, plus 1 cc of 0.25% preservative-free Marcaine. The patient was taken to the recovery room in good condition. Her vital signs were stable. Her right neck pain was improved. She is having some soreness related to the injections, but otherwise was doing well.

She is scheduled to return in 48 hours to undergo a left cervical facet block and possibly some trigger point injections into her neck region.
